SBGHC – Chesley Site
- 5 bed unit focused on helping patients regain their independence after the acute phase of an illness with the expectation that they will be able to return home.
- For suitable candidates, the program serves as an alternative option to applying for placement in a Long Term Care facility.
- Patients care for themselves (dressing, bathing, taking medication) with assistance if required, and participate in daily activities designed to regain strength and independence.
- The support team includes an Occupational Therapist, Physiotherapist, Nurses, Personal Support Workers (PSW) and an Occupational Therapy/Physiotherapy Assistant.

How do I access this service?
Patients from all of the hospital sites in Grey and Bruce Counties are screened by Community Care Access Centre (CCAC) Case Managers for program eligibility.
